Mangalam Laboratory is a computerized medical diagnostic laboratory located at Harimadhav Complex, Anandpur Road, Chotila, Ta. Chotila, Dist. Surendranagar, Gujarat 363520, operated by Bhanaji V. Kukadiya (B.Sc. Micro, P.G.D.M.L.T.). This policy explains how we handle the personal information you share with us through this website, by phone, or on WhatsApp.
2. Information We Collect
When you book a test or request a report through this website, we collect:
- Full name
- Mobile number
- The test or test category you want to book (health-related information)
- Preferred appointment date
- Home address (only if you request home blood collection)
- Any notes you choose to add to your booking
We do not collect payment information on this website, and we do not use tracking or advertising cookies.
3. Why We Collect It
We use this information only to:
- Process and confirm your test booking
- Contact you about your appointment or home collection visit
- Prepare and deliver your test reports
- Respond to your questions and report requests
4. How Bookings Are Stored
Booking details submitted on this website are stored in a private Google Sheet (Google Workspace) accessible only to the laboratory, and a booking summary is also sent to the laboratory's WhatsApp number so we can respond quickly. Both services are used solely for managing bookings. Communication over WhatsApp is protected by WhatsApp's end-to-end encryption.
5. Data Sharing
We do not sell, rent, or trade your personal information. We do not share it with any third party for marketing. Your information is only visible to laboratory staff who need it to serve you, and to the service providers named above (Google, WhatsApp) strictly as data processors for booking management.
6. Data Retention & Deletion
We keep booking records only as long as needed for patient service and basic record-keeping. You may ask us at any time to see the information we hold about you or to have it deleted. To request deletion, call +91 96240 00187 or email kukadiyabhanaji@gmail.com, and we will remove your records within 7 working days unless the law requires us to keep them.
7. Security
Access to booking data is limited to authorized laboratory personnel. We use reasonable technical and organizational safeguards appropriate for a small medical laboratory to protect your information from unauthorized access or disclosure.
8. Children's Information
Bookings for children must be made by a parent or guardian. We collect a child's details only as part of a booking made by an adult responsible for them.

If we update this policy, the new version will be published on this page with a revised effective date.
